For nearly 60 years, Calvary Healing Center has provided a full continuum of care, specializing in addiction treatment for adults across the Phoenix area and Nationwide. Calvary’s proven treatment programs address the practical, physical, and spiritual aspects of addiction for lasting recovery. Our flagship location in Phoenix offers onsite detoxification and residential treatment programs, a partial hospitalization program (PHP), and an intensive outpatient program (IOP). We also have a satellite clinic offering Intensive Outpatient Programs in the West Valley region. Summary of Duties

The Chaplain functions as an active part of the patient treatment team, providing patient care, supervision, interaction, and role modeling to patient. The chaplain utilizes clinical and pastoral skills to assist the treatment team in giving care to patients. Special focus is given to developing the religious resources of each patient in the healing process.

Qualifications

Chaplain Job Requirements:

+ Bacherlors degree from an accredited college or university in Biblical or Religious counseling/studies

+ At least one year of related experience, with a knowledge of psychiatric patient care techniques with understanding of mental illness and chemical dependency (12 -step program), preferred.

+ Must have a valid driver's license and agree to a Motor Vehicle Driving Record check as part of the background check. And that record must be in good standing.

+ Completion of BLS & First Aid Certification within 30 days of hire.

+ Completion of Crisis Prevention and Intervention (CPI) Certification within 30 days of hire.

The ACT Employment Specialist engages members and provides direct treatment, rehabilitative and supportive services in a community setting. Conducts vocational assessment, provides direct job development and placement services in addition to benefits counseling and employment coaching support services to assist to obtain and maintain community-based employment. Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage.

The ACT Employment Specialist duties include:

+ Facilitates educational and supportive activities including orientation into the work environments, the effect of work and meaningful community activities on benefits programs, job seeking skills, benefits counseling, job clubs, and employment support groups.

+ Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a client advocate and liaisons with families and community agencies.

+ Serves as the team’s specialist for employment services by collaborating with other team members in the assessing for and delivery of employment skills training as specified in the individual service plan.

+ Provides on-call crisis intervention services, as well as weekend and holiday coverage.

Qualifications

+ Bachelor's degree in Behavioral Health or related field or a combination of experience and education.

+ Three years experience in the delivery of behavioral health services required.

+ A minimum of one year of vocational rehabilitation training or supervised experience as a Rehabilitation or Employment Specialist, or Vocational Rehabilitation Counselor required.

+ Three years working as an employment specialist preferred.

+ Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.

+ Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.

+ Basic understanding of the DSM-V.

+ Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.

+ Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy

Schedule: 4 - 10's, Tuesday-Friday

Offering an EXCELLENT SALARY ($73K-$85K+ depending on years of experience) + ACT Differential Pay (+ $2/hour = $4160 approximately)

The ACT RN provides direct clinical services to an individual, group and/or family members in community based settings. Community visits include but are not limited to homes, jails, hospitals, etc.

+ Coaches members and families on behavioral and physical health conditions, self-management techniques.

+ Promotes treatment adherence and personal wellness within a recovery framework.

+ Coordinates member care within the clinical team and with external parties including the member’s primary care physician at a minimum of monthly and ensures current medical mediations are documented in the Medication module.

+ Attends Primary Care and Specialty appointments with the client.

+ Demonstrates critical thinking and takes initiative and demonstrates the ability to work independently within the scope of practice.

+ Performs health evaluations (HRA), takes vitals, draws blood for lab tests ordered, assists with medication monitoring, administration of intramuscular medications and coordinates care with other providers in support of the integrated health model.

Duties/Responsibilities Include:

+ Responsible for the oversight and monitoring of medication delivery, observation and administration for the ACT Team clients. Keeps a log of all patients receiving medication to ensure that each patient is receiving medication as ordered.

+ Actively participates as part of the member’s clinical team.

+ Takes full responsibility for the integrated treatment services for members coordinates care with ACT team members and other providers to ensure member’s needs are met and the member continues to receive treatment in the community to the greatest extent possible.

+ Works with informal support system with or without the member present to provide clinical skills training to increase informal supports, family, landlords, employer etc.)

+ Has a thorough understanding of ACT fidelity measures and maintains compliance

+ Utilizes assertive engagement mechanisms, including street outreach and legal mechanisms (probation/parole, court ordered treatment), when necessary, to re-engage the member in prescribed treatment.

+ Routinely assesses member’s need for ACT services, using explicit criteria to determine need to transfer to less intensive services. Gradually and individually transitions eligible members and ensure continuity of care. Monitors member’s status during transition and reintroduces services as needed to ensure member’s stability in the community.

+ Is responsible for crisis services for assigned members, coordinating hospital admissions and discharge planning.

Qualifications

INDUSTRY: Nursing

EDUCATION: AA, BS or MA in nursing

JOB SPECIFIC REQUIREMENTS: Registered nurse with the Arizona State Board of Nursing. One (1) year prior experience in psychiatric setting and/or working with members receiving behavioral health services; experiencing working within psychiatric crisis and/or with members with serious mental illness preferred.

KNOWLEDGE, SKILLS, AND ABILITIES: Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members. Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions. Strong understanding of substance use, addiction and recovery principles. Knowledge of chronic conditions experienced by those with serious mental illness preferred.

**Responsibilities**

**SUMMARY** - Plans, administers, and conducts the programs and services of Clinical Pastoral Education (CPE) program in accordance with the mission and vision of Dignity Health and in collaboration with the

Spiritual Care Team. An incumbent has responsibility to ensure the CPE program maintains its national accreditation. Work also includes oversight for program budget and planning, directing and evaluating

the work of subordinate staff, students and interns. Work requires a thorough knowledge of the national accreditation standards relating to CPE programs as well as a thorough knowledge of planning, developing, and evaluating clinical pastoral education programs and services.
