**This is a Full Time position working in the Inpatient department at Northwest Medical Center.**

**Seeking a candidate who is dedicated to quality patient care across the continuum, working in an acute care setting. Join a collaborative rehabilitation department dedicated to an interdisciplinary and transdisciplinary team-based approach to medicine. In this department, there are opportunities to cross train in other levels of care across Northwest Medical Center locations including acute care, inpatient rehab, skilled nursing, outpatient, and NICU.**

**Job Summary:**

Evaluates and assesses the needs of patients and formulates treatment plans. Develops, carries out, and documents treatment programs. Provides education and training for patients, caregivers, and families. Coordinates within the interdisciplinary team including (but not limited to) nursing, physicians, specialists, respiratory therapy, dieticians, case management, ST/OT, radiologists, etc. Considers, plans, and coordinates ongoing treatment plans to facilitate safe and effective discharge. May supervise physical therapy assistants, aides, and athletic trainers. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Reports to the Director of Therapy Services.

**Position Qualifications/Minimum Requirements**

**Education: Required:** Bachelor/Master/or Doctorate degree from an accredited program in physical therapy.

**Experience: Preferred:** One (1) year in acute care

**Skills:** Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.

**Licenses/Certifications: "Required:** Current Arizona Physical Therapist license;

American Heart Association (AHA) or American Red Cross(ARC) or American Safety and Health Institute (ASHI) BLS certification

**Skilled Nursing Unit:** Fingerprint Clearance Card "

**Work Schedule:**

This is a full time, flexible work schedule. The position is 5 days per week, which includes two weekend days per month. Option for four, 10 hour day shifts to be negotiated.
