With eleven LDR rooms and 2 OR suites we have approximately 320 deliveries per month and serve moderate or low risk patients. As a **Birth Registrar** , you will be responsible for assisting new parents in completing the Birth Certificate packets for their new arrivals! You will be assisting in completion of these packets by compiling health history, prenatal care information and education resources. This role is perfect for those who possess excellent customer service skills combined with great ability to multitask and pay attention to detail. **This is a part time shift position working 3 days per weeks with weekend rotation.**

Banner MD Anderson Cancer Center Located in Gilbert, Ariz. (the Phoenix Metro area) on the Banner Gateway Campus, the center provides world-class care for oncology patients - both inpatient and outpatient - and has also brought leading oncology programs to the Banner Gateway campus including designation as a Stem Cell Transplant Center of Excellence and comprehensive Head & Neck cancer care. Our capabilities include five linear accelerator vaults, a brachytherapy vault, an advanced diagnostic imaging suite with PET/CT scan, more than 70 infusion bays, a cryopreservation lab and much more. Our inpatient medical oncology unit also incorporates a program that utilizes the electronic surveillance partnership in caring for the patient, where remote nurses have the ability to interact with patients via two-way audio-video to assist the bedside nurse with patient care.
POSITION SUMMARY
This position is responsible for completing birth certificates as required by the state using the electronic birth certificate programs. The position is also responsible for completing paternity affidavits, fetal death certificates, death certificates, and maintaining pertinent hospital records. Notarizes documents as needed.
CORE FUNCTIONS
1. Interviews parents and reviews medical record to correct information for birth/death certificates or paternity.
2. Educates parents on state vital statistic legalities and processes with regards to birth, death, social security and paternity.
3. Records birth and death certificates and reports per state guidelines.
4. Notarizes and records documents following Arizona state laws.
5. Assists parent in accurately completing paternity affidavits assuring compliance with Arizona state laws.
6. Maintains accurate hospital records (i.e. birth certificate worksheets, baby name up-dates).
7. Customers are patients, family members, physicians, other BHS staff, representatives from the State office, and other hospitals.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge. Must have basic computer skills. Good written communication, organizational skills and be detailed oriented.
PREFERRED QUALIFICATIONS
Medical terminology. Bilingual (English/Spanish) strongly preferred.
Additional related education and/or experience preferred.
