**Overview**
Dignity Health Connected Living has been serving seniors in Shasta County since 1968. Our programs have been designed for seniors and people with disabilities. We are dedicated to meeting each participants physical mental and spiritual needs and we offer a variety of high-quality affordable health care services in a compassionate environment.
**Responsibilities**
This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting this individual assists clients in overcoming barriers to health social services education and employment by connecting them to identified Pathways. The incumbent empowers clients to navigate medical behavioral health and social services by removing barriers to these services.
**Serving Mt. Shasta Tuesday-Friday and Redding on Monday.
**Qualifications**
**Minimum Requirements:**
+ One (1) or more years of experience in social services or care coordination specifically serving older adults' behavior health clients or adults with complex chronic conditions
+ CA Drivers License
**Preferred Requirements:**
+ Community health worker training or certification
+ Bilingual/bicultural Spanish
**Pay Range**
$23.00 - $24.72 /hour
