**Overview**
Dignity Health’s Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast’s award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.
**Responsibilities**
**The incumbent is accountable for the day to day planning and care coordination provided to patients receiving surgical medical in-patient or outpatient healthcare services in Cancer Care.**
**Qualifications**
**Minimum**
+ Required training or certification in patient navigation or willing to successfully complete training in patient navigation (required).
+ Registered Nurse with 2-3 years experience in cancer care and treatment (required).
+ Current ONS Chemotherapy/Immunotherapy certification required.
+ RN
+ Special skills required include: good presentation and computer skills; experience in educating and/or navigating adults one-on-one and in larger groups; comfortable in dealing with death and dying; ability to deal with people in extremely stressful situat
